Form templates

The form template gallery has ready-made forms for contact, feedback, RSVPs, event registration, job applications, orders and more, each already laid out with sensible questions.

Use a template

Open Form Library in the menu, pick one, and click Use this template. Or, in My Forms, click New form and choose Start from a template.

A copy of the form is added to My Forms as a draft, so you can change, reorder or remove any question before you publish. Nothing is shared until you publish.

Other ways to start

When you click New form, you can also choose:

• Start blank: an empty form you build question by question.
• Generate with AI: describe the form and the AI drafts it for you.
• Form for a document: starts from one of your document templates, so each response generates a document.

All of them open the same builder.
